Upcoming closures for Churchfields Medical Practice

NHS Dentistry 

Access to NHS Dentistry is currently limited during the pandemic as all dentists have been asked to stop all routine “face-to-face” dental care.

However, if you need urgent dental care, help is at hand:

They will give you advice over the telephone and make arrangements for you if you need to be seen.

Emotional and psychological support 

For those in need of emotional and psychological support at this difficult and challenging time, Trent PTS is continuing to provide a service for all patients registered with a Greater Nottingham GP. This is NHS funded.  If you would like psychological support anybody aged 18 years or over can self-refer. Please go to our website to complete a referral and we will be in contact to arrange your first appointment.

As we continue to accept new referrals we are now offering our services by phone, webcam, using online text-based therapies or through online support packages so as to maintain social distancing.

Changes to the appointments booking service

Following developments of the Coronavirus pandemic and in the interest of patients and staff safety we have made the difficult decision to suspend the booking of all routine appointments for the time being.

We will be operating a telephone triage system and will be still offering telephone, video consultations and emergency appointments.

There will also be limited access to the practice by keeping the main doors closed. People attending the surgery for an appointment will need to press the intercom for access.

There will be a letter box where you need to put your prescriptions requests. If you normally collect it from the surgery, please use the intercom to advise the receptionist which pharmacy you'd like to collect it from when it's ready. 

These decisions have been made following advice from Public Health England. Thank you for your understanding.

We are pleased to say that our surgery is now live on the NHS app. Please follow the simple instruction to set it up.
You can book all non-urgent GP appointments, telephone consultations and some nurses appointments (smears, removal of sutures/clips,diet advice).

GP+ Nottingham City – extended hours access for patients

 

Offering Evening & Weekend Appointments bookable through your GP practice, patients registered at this practice are able to access additional routine appointments during evenings and weekends through the new GP+ service.

 

Appointments are available to see GPs, Practice Nurses, Clinical Pharmacists and Physiotherapists in a fully equipped accessible location on Upper Parliament Street in Nottingham City Centre.

 

Opening hours are:

 

16:00-20:00 Monday – Friday

9:00-13:00 Saturday & Sunday

NHS HEALTH CHECKS
The Practice offers free NHS health checks for patients age 40-74. Letters will be sent out shortly to those eligible to make an appointment with nursing team.

NHS BREAST SCREENING
Eligible patients registered at Churchfields Medical Practice are being invited for routine breast screening throughout 2017 by Nottingham City Hospital Breast Screening Unit. Over the next few months, women aged 47-73 will be receiving an invitation to be screened. More information regarding the screening can be found at www.cancerscreening.nhs.uk

 

National Diabetes Audit - this practice is taking part in an important national project about diabetes care and treatment in the NHS. Theis project is called the National Diabetes Audit (NDA). We will send anonymous information about the care and treatment received by our patients with diabetes. This information is strictly controlled. If you do not want your information used, please inform a receptionist. This will not affect your care. You can get more information from www.hscic.gov.uk/nda
 
Information sharing agreement (MIG) - The practice has enabled software to share your GP medical record with urgent care services. This means that if you need urgent care treatment (A&E, Out of hours GP, Walk-in centre) the clinician treating you will ask for your consent for them view your GP medical record; if you do not wish them to view your GP medical record you can decline access. This will only work if you have provided us with consent to share information from your record with other healthcare providers - please click on the 'Enhanced Data Sharing Model' heading below for more information and a consent form.

The type of information shared is restricted and includes a summary of current problems, current medication, allergies, recent tests, diagnosis, procedures, investigations, risks and warnings – all this information is currently held in your GP system record.

Family & Friends test - The feedback we're getting from the Family & Friends test is mostly very positive - thank you. We've also had some helpful suggestions for improvement, that we're looking into. You can complete the Family & Friends test online (the patient survey button at the bottom of the home page) or in the practice (feedback box). We also send text messages after your appointments here giving you the option to share your feedback.

This practice currently offers patients’ facilities to book, view, amend, cancel and print appointments online.We also offer the facility for all patients to order online, view and print a list of their repeat prescriptions for drugs, medicines or appliances. We are also now able to to offer patients facilities to view online, export or print any summary information from their record, relating to medications, allergies and adverse reactions. We will need to see photo ID to set these up - please speak to a receptionist for more information

Welcome to Churchfields Medical practice

Churchfields Medical practiceChurchfields doctors' surgery in Old Basford provides the highest quality healthcare to patients in the Nottingham area.

Here for you

Our team includes 3 GP Partners, 2 salaried GPs, 2 practice nurses, 1 minor illness nurse, 2 health care assistants, practice pharmacist, secretarial and reception staff. We're working closely with other health and social care providers in the area, as part of a care delivery group, and have great links to other healthcare professionals such as district nurses, health visitors, a community matron as well as community paediatricians and a community diabetic nurse.

Safe and Good Quality Care

The practice was inspected by the Care Quality Commission (CQC) in December 2017and we were rated as "GOOD" in all areas. The inspection report is available on the CQC website - here

Training for the Future

Churchfields Medical practice is a “training practice”. This means we train fully qualified Foundation doctors and GP Registrars, who are attached to our practice for a period of 4 months whilst working closely with the rest of the team. This includes regular reviews of their consultations and record keeping.

Please have a look around and do send us some feedback if you like. We can use your ideas to improve our on-line services and further develop the content of this site.

Feedback from patients

We value the feedback our patients have to share, and gather it from a range of sources. There are always feedback forms in reception, you can speak to us directly and feedback can be submitted via this website. There are also a number of more formal patient surveys completed each year, by both the practice and external organisations. When the practice completes a survey, we will publish the feedback on this website and in the practice newsletter. To see the results of the main external survey, click here. If you wish to, you can also share your feedback with the public on the NHS Choices website.

The Friends & Family test is another way of gathering patient feedback. This can be completed in the practice, by SMS response to our request or on this website - through the patient survey button below. We would like to hear the views of all of our patients, after every interaction with the practice. If you have difficulty completing the survey, you can ask someone to help you.

Since 1st January 2016, we have had 8897 responses to the Family & Friends test. This is a small proportion of the interactions we have had with patients (appointments, repeat prescriptions etc). We encourage all patients to share their thoughts with us. Here is a summary of the responses for this period.
