The Practice will closed from 12pm on Tuesday 19 March 2019. There will be no baby immunisations clinic on that day.

Changes to appointments
In the upcoming weeks we will be introducing some changes to the way routine appointments are being booked. When calling the practice to book a routine appointment, the receptionists will ask you the reason for the appointment in the same way they already ask you when booking an urgent or nurse appointment. This is so we can signpost you to the most appropriate clinician which will make it easier for you to access the help that you need. This information is being asked at the request of the doctors and clinicians working at the practice.
It would also be a great help when booking a telephone consultation or an utilising the online booking system that again you provide a reason for booking your appointment.
Please be assured that ALL staff working at Churchfields Medical Practice have signed a confidentiality agreement and any information given will be treated in the strictest confidence.
FLU VACCINATION CLINICS (drop in clinics every Tuesday 1pm - 2pm)
The vaccination is free and recommended yearly for those most at risk from flu.
- pregnant women
- anyone living with a long term medical condition (diabetes, heart disease, COPD, asthma etc).
- everyone aged 65 and over
- obesity (BMI >40)
Children Immunisation Clinic
From Tuesday 7th August, we'll be running Children Immunisations Clinic.
This will be a drop in clinic every Tuesday 1pm-3pm and will run at the side of the Child Health clinic. Places will be limited but please ask at reception.
GP+ Nottingham City – extended hours access for patients
Offering Evening & Weekend Appointments bookable through your GP practice, patients registered at this practice are able to access additional routine appointments during evenings and weekends through the new GP+ service.
Appointments are available to see GPs, Practice Nurses, Clinical Pharmacists and Physiotherapists in a fully equipped accessible location on Upper Parliament Street in Nottingham City Centre.
Opening hours are:
16:00-20:00 Monday – Friday
9:00-13:00 Saturday & Sunday

NHS HEALTH CHECKS
The Practice offers free NHS health checks for patients age 40-74. Letters will be sent out shortly to those eligible to make an appointment with nursing team.
NHS BREAST SCREENING
Eligible patients registered at Churchfields Medical Practice are being invited for routine breast screening throughout 2017 by Nottingham City Hospital Breast Screening Unit. Over the next few months, women aged 47-73 will be receiving an invitation to be screened. More information regarding the screening can be found at www.cancerscreening.nhs.uk
National Diabetes Audit - this practice is taking part in an important national project about diabetes care and treatment in the NHS. Theis project is called the National Diabetes Audit (NDA). We will send anonymous information about the care and treatment received by our patients with diabetes. This information is strictly controlled. If you do not want your information used, please inform a receptionist. This will not affect your care. You can get more information from www.hscic.gov.uk/nda
Information sharing agreement (MIG) - The practice has enabled software to share your GP medical record with urgent care services. This means that if you need urgent care treatment (A&E, Out of hours GP, Walk-in centre) the clinician treating you will ask for your consent for them view your GP medical record; if you do not wish them to view your GP medical record you can decline access. This will only work if you have provided us with consent to share information from your record with other healthcare providers - please click on the 'Enhanced Data Sharing Model' heading below for more information and a consent form.
The type of information shared is restricted and includes a summary of current problems, current medication, allergies, recent tests, diagnosis, procedures, investigations, risks and warnings – all this information is currently held in your GP system record.
